Membership Policy
- Your monthly payments will be taken on an agreed date each month, and the name Bespoke Smile Ltd will appear on your bank statement
- Your benefits will commence following your first monthly payment
- You will be asked to remain a member for a minimum of 12 months
- If your Direct Debit payments fail twice consecutively, we may cancel your membership
- Patients are responsible for making and attending appointments; missed appointments still carry the same penalties as non-member patients
- Entitlements run from the date of commencement for 12 months and cannot be carried over to the following year
- Your membership auto-renews unless you discuss changes to your plan with your dentist
- In the unlikely event of you cancelling your membership, you may be required to pay for your last appointment, and we reserve the right to recover any discounts applied to treatment completed in the previous 6-month period
